What Is the I-693 Medical Exam?

Form I-693, officially called the Report of Immigration Medical Examination and Vaccination Record, is used by USCIS to determine whether an applicant meets the health-related requirements for permanent residence.

The examination generally includes:

  • Review of your medical history

  • Physical examination

  • Review of vaccination records

  • Required laboratory testing based on age and current guidelines

  • Completion of Form I-693

  • A sealed medical examination packet for USCIS

  • A copy of the completed form for your records

What Is Included?

The examination fee includes:

  • Medical history and vaccination record review

  • Physical examination by the civil surgeon

  • Completion of the required I-693 paperwork

  • Final follow-up visit, if needed

  • Original completed form in a sealed USCIS envelope

  • A personal copy of the completed form

Laboratory tests, vaccinations, immunity titers, chest X-rays and additional medical evaluations are not included. If any additional services are required, our staff will explain them and the associated cost in advance.

What Should You Bring?

Please bring:

  • A valid passport or government-issued photo ID

  • Your vaccination records

  • English translations of records written in another language

  • Relevant medical records

  • Records of previous tuberculosis testing or treatment, if applicable

  • A list of your current medications

  • Any USCIS notice or instructions related to your case, if available

  • The current edition of Form I-693

You may complete the applicant information in Part 1 before your appointment, but do not sign Form I-693 until instructed by the civil surgeon.

Parents or legal guardians should bring identification and available medical and vaccination records for minor children.

Completed USCIS Packet

After all requirements are satisfied, you will receive the completed Form I-693 in a sealed envelope for submission to USCIS, along with a copy for your records.

Do not open the sealed envelope. USCIS may not accept a packet that has been opened or altered.
