A valid passport or government-issued photo ID

English translations of records written in another language
Relevant medical records
Records of previous tuberculosis testing or treatment, if applicable
A list of your current medications
Any USCIS notice or instructions related to your case, if available
The current edition of Form I-693
You may complete the applicant information in Part 1 before your appointment, but do not sign Form I-693 until instructed by the civil surgeon.
Parents or legal guardians should bring identification and available medical and vaccination records for minor children.
